| DAV Officer Report (from National) |
For your convenience, we have posted the revised (2/16) DAV Officer Report form required by National when new Officers are installed by each Chapter or if there is any change in Officers. The normal form is a 4-piece, multi-colored, carbon copy type form, with each colored section being sent to the proper section as follows: White copy-National Headquarters; Yellow copy-Department Headquarters; Pink copy-New Orleans National Service Office; Green copy-Chapter copy.
We have posted a pdf of each of the form sections in their original color and suggest to print them in color if possible. Please be sure to complete ALL four sections of the DAV Officer Report.
